Focused Assessment (FA)

Focused Assessments take approximately 60 days to complete, depending on the questions answered.   If you believe such an evaluation may assist in your case, please contact Dr. DiCarlo to determine availability and time frames. It will be necessary to obtain an Order of Appointment from the Court before beginning.


Step 1

 

Complete Initial Documents. If Dr. DiCarlo has already been appointed to conduct your evaluation, you can download and complete the initial documents here. Each party will need to submit a completed and signed consent form and background questionnaire, which can be emailed directly to Dr. DiCarlo.

PLEASE DO NOT SUBMIT ANY ADDITIONAL DOCUMENTS SUCH AS EMAILS, TEXT MESSAGES, RECORDINGS, ETC. TO YOUR SUBMISSIONS AT THIS TIME. THESE MAY BE REQUESTED LATER DURING THE PROCESS.

 

If you prefer to complete your background questionnaire using a word processor, you may download the MS Word version here. Please do not remove any of the questions or fields. Any substantive modifications to the document in order to omit requested information will be noted in the evaluation report.


Step 2

 

Submit Payment. The Advance Fee (retainer) is $4000.  The Order Of Appointment will specify the fee split for the parties.  Each responsible party should pay their initial designated portion of the advance fee. Payments can be made by check, cash, or securely online by all major credit cards here:


Step 3

 

Attorney Call. Once the above steps are completed, Dr. DiCarlo will request a call with your attorneys (if represented), in order to discuss the circumstances of your case.


Step 4

 

Initial Appointments. The evaluation begins with an initial interview of each parent or litigant in the case, at which time the evaluation process will be explained in further.
